The Oak Ridge office will be doing birth certificates Monday thru Friday. The office is located at 728 Emory Valley Road. The cost of a birth certificate is $13.00 dollars, and you are required to bring ID. There is a form that you can download, print and fill out and bring to the office that will expedite the process. Click here for form. We can only print for years 1949 to the current year. Parents names will not appear on the short form printed in the Clerk`s Office from 1949 to 1976. For parents name to be shown you will need to apply with the Department of Health on a long form. There is a 4:45 pm cut off time for printing birth certificates.
